Common Security Forms

Administrative and Root Access Request
- Used for requesting admin or root access to systems, such as for managing a lab.
MOAT Account Reinstatement Form
- Required for account reinstatement for accounts disabled due to not completing MOAT security awareness training before the expiration date.
Special Data Audit Request
- Used to request access to another user's files or email. This is most commonly used when an employee has separated from the university and another user needs to continue their work.
VPN Access Request
- For faculty and staff to request access to NSU's sanctioned and managed VPN service. All other VPN services are prohibited per the #32-01 - Acceptable Use of Technological Resources policy.
