Faculty and Staff Account Requests (New, Returning, or Changes)
In order for this IT service request to be processed, you must attach a fully completed Information Security Access Agreement (ISAA). This includes both Page 1 and Page 2 (Sections 1 and 2). Incomplete forms will not be accepted—the ISAA must also be signed by an HR Business Partner.

New Accounts and Rehires: In addition to the ISAA, you are required to complete the Security Awareness Training and provide the OIT Information Security Group Certificate of Completion. Training can be accessed at: https://webapps.nsu.edu/securityawareness/newusers.
